Acupuncture Patient Visit pointers

Hi, and welcome to my practice!!! Here are few helpful pointers for your first acupuncture visit.

All Patients: Please fill out initial intake and paperwork ahead of time (these are available for you to download from our website): All Patients required form

If you are coming to see us for women’s health issues,

1. Fill out the New Patient General Health
2. Fill out the first two pages of our
3. Women’s health new patient form.

If you are comming in for fertility treatments, please

1. Fill out New Patient General Health
2. Fill out the complete 3 pages of our Women’s health new patient form.

Men comming in for fertility treatment.

1. Fill out New Patient General Health
2. Fill out our Men’s fertility patient form

Make sure that you have eaten a snack or meal within 4 hours of your appointment. Please do not come in on an empty stomach.

Wear comfortable clothing that can easily be pulled above knees and elbows

RELAX!!!….the needles are tiny.
